Most evacuation diagram audit failures in Australia are not exotic technical defects. They are the same handful of mistakes, made over and over, by building owners and managers who inherited a stale set of diagrams or commissioned them from a template-overlay supplier years ago. Every fire safety practitioner who signs an Annual Fire Safety Statement, every council inspector running an occupancy check, every Fire and Rescue officer responding to a complaint walks in with the same short list of things they check first.

This post lists the 12 most common reasons an evacuation diagram fails an AS 3745 audit, with the clause reference, why the inspector flags it, and the fix. Each item is something the building owner can audit themselves, on a single walk-through, before an inspection lands.

How inspectors actually assess diagrams

Before the list, a quick note on the inspector mindset. Inspectors are not looking for theoretical perfection. They are looking for whether the diagram, mounted in this location, in this state of currency, in this lighting, would actually help an occupant evacuate. That is the single mental model they apply. Everything else (the clause numbers, the size minimums, the orientation rules) is the standardised way of asking that question.

When you do your own audit walk-through, stand in front of each diagram and ask: if this building filled with smoke right now, would this diagram tell me what to do, where to go, who to call, and what to look for on the way? The mistakes below all break that test.

1. The "You Are Here" marker is wrong

This is the single most common failure. Someone moved the diagram, or copied a diagram from one location to another, and the "You Are Here" marker now points to the wrong spot. An occupant standing at the entrance, looking at a diagram that says "You Are Here" near the back hallway, will misread every exit route on the page.

AS 3745:2010 is explicit: the pictorial representation of the building must include the location of the diagram itself, accurately. Inspectors check this by physically standing where the diagram is mounted, looking at the marker, and verifying it matches.

Fix: every diagram must be rendered for its specific mounting location. The same building floor needs multiple diagrams, each with its own "You Are Here" indicator. A single diagram copied to ten locations fails ten ways. If you are commissioning diagrams, ask the supplier how many "You Are Here" variants will be produced. The answer should match the number of mount locations.

2. The diagram is a generic template, not site-accurate

Template-overlay diagrams are the second most common failure. A supplier takes a stock floor plan that approximates the tenancy, adds your business name and contact numbers, and prints it. The exit routes are inferred, not measured. Fire equipment locations are placed where they "usually" are. Manual call points are shown in plausible positions, not actual ones.

Inspectors detect this in two ways. The first is visual: a real floor plan has the asymmetries and quirks of a real building (an offset doorway, an unusual column placement, a non-rectangular meeting room). Template overlays look too clean. The second is verification: an inspector walks the building and finds a fire extinguisher marked on the diagram that is not actually mounted on that wall, or a manual call point in the diagram that does not exist on the ground.

Fix: the diagram must be drawn from your actual floor plan (architectural plans, tenancy plan, or a measured-up survey), with fire equipment locations verified on site. 3. The diagram orientation is wrong

AS 3745:2010 clause 3.6 requires that the diagram be oriented from the viewer's perspective. If the diagram is mounted on a north-facing wall, "up" on the diagram must correspond to whatever the viewer would see by walking straight forward from the diagram. North-up is not a default. Viewer-relative orientation is mandatory.

A common audit failure: a single diagram artwork is reproduced at four mount locations around a building, all rotated the same way (typically north-up). At three of those locations, the diagram is misoriented relative to the viewer, and an occupant trying to find the nearest exit will instinctively go the wrong direction.

Fix: each diagram must be rendered with its own orientation, matched to the mount location. The same floor in a building can need 4, 6, or 8 different diagram orientations, one per mount point. This is why a serious supplier produces a separate render per location, not a single artwork.

4. Mount height outside 1,200 to 1,600 mm

AS 3745:2010 clause 3.7.2 specifies that the diagram must be mounted between 1,200 mm and 1,600 mm above the floor, measured to the centre of the diagram. The range exists because diagrams mounted too low are blocked by furniture and not seen; mounted too high they sit above the eye line and the floor plan detail becomes hard to read in an emergency.

Inspectors carry a tape measure. They will spot-check 2 or 3 diagrams in a building. If even one is at 1,800 mm because someone mounted it above a doorway for "visibility," it is flagged.

Fix: install diagrams at 1,500 mm to the centre as a safe target. That gives 300 mm of margin on the high side and 300 mm on the low side. For accessible mounting (where wheelchair users are the primary audience, such as NDIS facilities or aged care), 1,200 mm to 1,400 mm is preferred. 5. The diagram has no validity date, or the validity date has passed

AS 3745:2010 clause 3.5.5(l) requires a validity date on the diagram, and FPA Australia guidance recommends showing a date of issue alongside it. The reason is simple: an undated diagram cannot be assessed for currency. An inspector looking at a diagram without a date does not know whether the building has been renovated since, whether the fire equipment locations are still accurate, or whether the contact numbers were last verified a decade ago.

The validity period is nominated by the Emergency Planning Committee (clause 2.2(d)), and FPA Australia guidance says it should not exceed 5 years, though it may be shorter. A diagram dated 2017 in a building inspected in 2026 is presumed invalid unless evidence of a validation pass is produced.

Fix: every diagram must show a clear validity date, ideally with a date of issue alongside it, in a font size legible at the mounted height. Re-issue before the validity date lapses (5 years at the most), and re-issue immediately after any renovation, tenancy change, or change to fire equipment locations. 6. The diagram is smaller than the AS 3745 minimum size

AS 3745:2010 clause 3.5.3 specifies size minimums. With only mandatory elements, the diagram must be at least A4 (210 x 297 mm) overall, with the floor plan graphic at least 200 x 150 mm (30,000 mm2). If optional elements are included (first aid kits, AEDs, disabled refuge areas, fire and smoke doors, warden intercommunication points), the minimum size increases to A3 (297 x 420 mm), with the floor plan graphic at least 300 x 200 mm (60,000 mm2).

A common failure: a building that needs disabled refuge or AED locations shown (which most modern commercial buildings do) is still using A4 diagrams. The inspector measures the floor plan graphic, finds it under 60,000 mm2, and flags the diagram as undersized.

Fix: default to A3 for any building with optional elements (almost all multi-tenant or larger commercial buildings). 7. A mandatory element is missing

AS 3745:2010 clause 3.5.5 lists the mandatory elements: the floor plan, the "You Are Here" indicator, exit routes, exit signs, firefighting equipment, manual call points, the assembly area, emergency contact numbers, building name and address, and the validity date. Missing any one of these is a fail.

The two most common omissions in the field: manual call points (the small red "break glass" panels for activating the fire alarm) and the assembly area. Manual call points get missed because they are easy to overlook during the floor walk for the diagram artist. Assembly areas get missed because some buildings genuinely have a poorly-defined assembly area and the diagram author chose to omit it rather than guess.

8. A fit-out or renovation completed without re-issuing the diagram

This is the silent failure mode. A tenant moves a wall, adds a meeting room, converts a storage cupboard into a server room, or relocates a fire extinguisher to a different bracket. None of those events trigger an automatic re-issue of the diagram. Six months later, the diagram on the wall no longer matches the layout the inspector walks through, and the entire diagram is invalid.

AS 3745:2010 requires the diagram to stay current: it must be re-issued whenever the layout, exit routes, or fire equipment locations change. The validity date printed on the diagram (5 years out at the most) is a backstop, not a substitute. Any material change to the building during the validity window triggers an immediate re-issue.

Fix: bake the diagram re-issue into the fit-out close-out checklist. Every fit-out, every tenancy change, every internal wall move, every fire equipment relocation should trigger a diagram update before the work is signed off. 9. The diagram is faded, damaged, or obscured

AS 3745:2010 implicitly requires that the diagram be legible (the standard would be meaningless otherwise). In practice, inspectors flag diagrams that are sun-bleached, water-damaged, dirty, scratched, missing corners, partially obscured by signage or posters, or hung at angles that make reading difficult.

The most common version: a diagram mounted near a window has faded to the point that the exit routes are barely visible. The diagram was technically correct when installed but is now functionally illegible.

Fix: substrate matters. Print onto UV-resistant material and mount in a frame with anti-glare glass if direct sunlight is an issue. 10. The assembly area shown no longer exists or is inaccessible

Assembly areas drift over time. The original assembly area was a section of car park; it has since been developed into an extension. The original assembly area was a grassed median; it has been re-landscaped into a planted garden bed. The original assembly area was a public footpath that has since been gated off.

When the diagram shows an assembly area that no longer functions as one, inspectors treat this as a serious failure because evacuating occupants may follow the diagram to a dead end during an actual emergency.

Fix: walk to the assembly area shown on each diagram and confirm it is still: accessible from all exits shown, large enough for the maximum building occupancy, free of vehicle conflict, and reachable without crossing dangerous infrastructure. 11. The diagram is mounted on a fire door

Fire doors are required to be self-closing and to seal against smoke under their own weight or a closer. Anything mounted on the door (signage, posters, frames) can prevent the door from closing fully, compromising the smoke seal. AS 3745:2010 clause 3.7.2 specifically prohibits mounting diagrams on fire doors.

A common failure: a building has a fire door at the entrance to a fire stair, and the diagram has been mounted on the door itself because the adjacent wall was busy with other signage. The mounting hardware (often a wooden frame or rigid acrylic) prevents the door from sealing.

Fix: never mount on fire doors. Use the adjacent wall, even if it requires moving other signage. If wall space is genuinely constrained, mount on a freestanding sign next to the fire door rather than on the door itself.

12. Missing diagrams at required locations

AS 3745:2010 clause 3.7.1 lists the locations where diagrams are required: at main entrances and exits, at fire stair entries on every level, at lift lobbies on every level, inside hotel rooms and apartment entry doors, and at other locations needed for effective evacuation.

The two most common omissions: fire stair entries (the inspector finds a fire stair with no diagram on its entry level) and lift lobbies on upper floors (the building owner installed diagrams on the ground-floor lift lobby but not on the 2nd, 3rd, 4th floor lobbies).

Fix: produce a diagram-location schedule before commissioning, listing every required location across every floor. Cross-check against the AS 3745 list. If you know an inspection is coming (NSW AFSS renewal, a council compliance audit, a fire authority follow-up), the 1-day walk-through below catches most of the 12 failures above before an inspector does.

  • Walk every floor and identify every diagram mount location. Confirm the count matches the AS 3745 requirement (every fire stair, every lift lobby, every main entry, plus any other location relevant to evacuation).
  • At each diagram, stand directly in front of it and verify the "You Are Here" marker matches the actual mount location.
  • Verify the orientation. The "up" direction on the diagram should match the direction you would walk if you stepped forward from the diagram.
  • Measure the mount height. The centre of the diagram should sit between 1,200 mm and 1,600 mm. Anything outside that range needs to be re-mounted.
  • Verify the date is on the diagram and is no older than 5 years. If the building has been renovated since the date, the diagram needs re-issue regardless of date.
  • Confirm the diagram size is correct for your element set. A3 if optional elements are shown, A4 only for the simplest sites.
  • Verify each mandatory element is present (floor plan, "You Are Here," exit routes, exit signs, fire equipment, manual call points, assembly area, contact numbers, building name and address, date).
  • Spot-check 3 fire equipment locations from the diagram against the actual building. Are the extinguishers and hose reels in the locations shown?
  • Walk to the assembly area shown. Is it accessible, sized for the building occupancy, and free of obstruction?
  • Verify legibility. Stand 1 m from each diagram and confirm exit routes, fire equipment, and contact numbers are readable. Faded or damaged diagrams need to be replaced before the audit.
  • Confirm no diagram is mounted on a fire door.
  • Confirm contact numbers on diagrams are still current (the building manager, the fire warden, the after-hours emergency contact).

When to engage a supplier for re-issue rather than touch-ups

If your walk-through surfaces more than 3 of the 12 failures, the cost-effective path is a full re-issue from a fresh master file, not patchwork edits to the existing artwork. Patchwork edits (updating one element, re-printing one location, fixing one "You Are Here") rarely save money once you account for the design time, the re-print, and the re-mount.

A full re-issue from a current floor plan, with all mount locations rendered to spec, is usually cheaper than 4 to 5 individual patches.
